The objective of this MQP is to recognize and eliminate the root causes of making defective parts at Company A. Improper selection of tools, cutting conditions, and fixtures as well as over machining, poor flow of information and the limited number of skilled machinists are some of the major root causes we identified. Value stream mapping and lean manufacturing are used to recognize and eliminate the root causes. Through cooperation with the workers and management at Company A, an effective solution for eliminating the making of defective parts is established.
